The NHS publishes this figure for week 16, about the size of an avocado. It is an average from patient guidance rather than a measurement of any particular pregnancy, and the measurement basis changes at week 20 - see the note below the dates.

11.6 cm and avocado-sized, with a nervous system now developed enough to drive the arms and legs properly. Hands can make fists. The face pulls expressions - smiles, frowns - with no muscle control behind them and no feeling attached, which is a strange thing to picture and entirely ordinary. There is an appointment this week, and it is the first proper check since booking. A midwife takes blood pressure and a urine sample, may listen to the heartbeat, and goes through the results of the blood tests taken at booking. It is short and it is routine, and the useful part is that it is the first slot in the calendar designed for asking things. Bring the list. On the same page, the NHS notes that the whooping cough vaccination is offered from week 16 onwards, with around week 20 being typical and week 32 the outer edge of the window. Gums that bleed when brushed and the occasional nosebleed also turn up around here, both driven by the same increase in blood volume that has been building since the first trimester.

What the NHS describes this week

  • The NHS puts the foetus at around 11.6 cm from head to bottom this week.
  • The nervous system has developed enough to move the arms and legs, and the hands can form fists.
  • The face makes expressions such as smiling and frowning, without muscle control behind them.

What happens around now

An antenatal appointment is scheduled at 16 weeks. The NHS uses it to check blood pressure and urine and to go through the blood test results from booking. The whooping cough vaccination can be given from week 16.

  • 16-week antenatal appointment - blood pressure, urine sample, booking blood test results (NHS)
  • Whooping cough vaccination - available from week 16, usually offered around week 20 (NHS)

On the measurement. The NHS publishes a length for each week from 4 to 40. Up to week 19 it is measured head to bottom (crown to rump, the measurement a scan takes while the legs are curled). From week 20 it is measured head to heel, which is why the figure jumps from 15.3 cm at week 19 to 25.6 cm at week 20 - the baby did not grow 10 cm in a week, the ruler changed. The NHS states the change on its week 20 page.
