The NHS publishes this figure for week 34, about the size of a cantaloupe melon. It is an average from patient guidance rather than a measurement of any particular pregnancy, and the measurement basis changes at week 20 - see the note below the dates.

Forty-five centimetres, and the NHS reaches for a cantaloupe melon. Testicles are descending into the scrotum where relevant. The head is settling into the pelvis in a process called engagement, and when it happens some symptoms improve while others get worse: breathlessness and heartburn often ease because there is less upward pressure, while urination becomes more frequent and walking becomes harder because there is more downward pressure. It is a redistribution rather than an improvement. There is an antenatal appointment this week - health review, bump measurement, birth preferences, mental health. Colostrum leaking is common by now and breasts may be noticeably larger again. The car seat is the job this week if it has not been done, and it deserves more attention than it usually gets: fitting one correctly is genuinely fiddly, a large proportion are fitted wrong, and many retailers will check it for free. The hospital will not discharge you without one, so this is not a task with any flexibility in it.

What the NHS describes this week

  • The NHS puts the foetus at around 45 cm from head to heel this week.
  • The testicles are descending into the scrotum where applicable.
  • The head is moving down into the pelvis in the process called engagement.

What happens around now

A 34-week antenatal appointment is scheduled, covering a health review, bump measurement, birth preferences and mental health. Engagement often eases breathlessness and heartburn while increasing pressure lower down.

  • 34-week antenatal appointment - health review, bump measurement, birth preferences, mental health (NHS)

On the measurement. The NHS publishes a length for each week from 4 to 40. Up to week 19 it is measured head to bottom (crown to rump, the measurement a scan takes while the legs are curled). From week 20 it is measured head to heel, which is why the figure jumps from 15.3 cm at week 19 to 25.6 cm at week 20 - the baby did not grow 10 cm in a week, the ruler changed. The NHS states the change on its week 20 page.
